SYARAT-SYARAT PENDAFTARAN HAK CIPTA (COPYRIGHT REGISTRATION REQUIREMENT )

§  Mengisi  formulir  pendaftaran, diketik  rangkap  2 (dua)

§  Melampirkan:

§  Surat  kuasa   khusus , apabila  permohonan   diajaukan  melalui Kuasa;

§  Contoh ciptaan  dengan  ketentuan  sebagai  berikut:

§  Buku  dan  karya  tulis  lainnya : 2(dua) buah  yang telah  dijilid dengan edisi  terbaik;

§  Program  komputer : 2(dua) buah  disket  disertai  buku  petunjuk  pengoperasian  dari  program  computer tersebut;

§  CD/VCD/DVD: 2  (dua)  buah  disertai  dengan  uraian

Ciptaannya;

§  Alat  peraga: 1 (satu) buah  disertai  dengan  buku

Petunjuknya;

§  Lagu: 10 (sepuluh) buah berupa  notasi dan  atau  syair;

§  Seni  lukis, seni  motif,seni batik, seni kaligrafi, logo  dan gambar: masing-masing  10 (sepuluh) lembar  berupa Foto;

§  Fotografi:  10(sepuluh) lembar;

§  Terjemahan: 2 (dua) buah  naskah  yang  yang  disertai  izin  dari Pemegang  hak  cipta;

§  Salinan  resmi  akta  pendirian  badan  hokum  atau  fotokopinnya  yang  dilegalisasi  notaris , apabila  pemohon Badan hukum;

§  Fotokopi  kartu  tanda  penduduk; dan

§  bukti  pembayaran  biaya  permohonan

§  bukti pengalihan  hak  cipta  tersebut (jika ada).


REGISTRATION REQUIREMENTS OF COPYRIGHT
*.. Fill out the registration form, typed copies 2 (two) attach:
*. Special power of attorney, if the petition diajaukan through a proxy;
*. Example creation with the following conditions:
*. Books and other writings: 2 (two) pieces that have been bound with the best editions;
*. Computer program: 2 (two) diskettes with the operation manuals of the computerprogram;
*. CD / VCD / DVD: 2 (two) along with a description
creation;
*. Props: 1 (one) accompanied by a book
instructions;
*. Song: 10 (ten) notation in the form of fruit and or poetry;
*. Painting, art motifs, batik art, calligraphy, logos and images: respectively 10 (ten) copies of photos;
*. Photography: 10 (ten) sheets;
*. Translation: 2 (two) pieces of text that accompanied the permission of the copyrightholder;
*. Official copy of deed or legal entity which has been approved fotokopinnya notary, if the applicant legal entity;
*. Photocopy of identity card; and
*. proof of payment of application fee
*. evidence of transfer of copyright (if any).
